Dual Enrollment Parent/Guardian Authorization

Dual enrollment students are required through Title 5 § 56700 to submit one Parent/Guardian Authorization Form allow enrollment into a any Fullerton College Dual Enrollment course for the duration of their high school years.

This form only needs to be submitted one time and is stored in the student's academic record.


Please Note:

Dual Enrollment student education records are considered confidential. Parents or guardians may not obtain student academic or financial records without the signed written permission from the student as stated by the Family Educational Rights and Privacy Act (FERPA). Educational Records are defined as those records that contain information directly related to a student and which are maintained by an educational agency or institution or by a party acting for the agency or institution.

Parents and Guardians will have to create a NextGen account to complete the form.
