Registration & Enrollment
- Affidavit of Parent/Student Status Confirm your parent/student status for priority registration eligibility. Accessed through the Priority Registration Petition page.
- Dual Enrollment Parent/Guardian Authorization Form One-time authorization for a parent or guardian to enroll a student into Fullerton College Dual Enrollment classes. English and Spanish versions available on the landing page.
- High School Status Change Form Request to change your enrollment status from high school student to college student. Requires myGateway sign-in
- Academic Restart (Petition for Readmission) Submit a petition to be readmitted to Fullerton College following academic pause (dismissal). Requires myGateway sign-in
- Petition to Repeat Course Request to repeat a course for the current and upcoming terms. Requires myGateway sign-in

Grades & Academic Standing
- Academic Renewal Request to exclude substandard (D or F) grades from your GPA without repeating the course. Available via myGateway sign-in or Dynamic Forms account.
- AP/CLEP/IB Credit Petition Request to apply AP, CLEP, or IB exam credits toward your academic record. Requires myGateway sign-in
- Course Repeat Adjustment Request (Appeals Petition) Students who have retaken a course can request to exclude prior substandard (D or F) grades from their GPA. Requires myGateway sign-in

Special Programs & Eligibility
- Cross Enrollment with Cal States & UCs Allows Fullerton College students to take a class at a CSU or UC campus. Visit the info page for eligibility details and to access the PDF form.
- Special Admit Form Apply for special admission as a current high school student. Students in grades 9–12 submit the online form (requires myGateway sign-in). Students in grades K–8 submit the online form available on the landing page.
