Creating a NextGen Dynamic Forms Account

Non-student users — including parents, guardians, and academic partners — need a NextGen account to access Dynamic Forms. This is a quick two-step process.

Select a language, then click Translate. To return to English, refresh the page.

This is a two-step process: first create your account, then activate it by clicking a link sent to your email address. This verifies that you are the owner of the email provided.

Visit dynamicforms.ngwebsolutions.com and click the Create New Account link in the login panel.

Complete all fields on the Create a new account page, then click Create Account.

After submitting the form, an activation email will be sent to the address you provided. Open the email and click the Activate your account link.

After clicking the activation link, you will be automatically logged in. Click Continue to proceed to the form you requested, or to the basic user home page if you did not arrive from a form link.

You can return to dynamicforms.ngwebsolutions.com at any time to retrieve your username or password, or to view your completed forms.
